Filling out the order form
Fill out your complete name, address and the telephone number(s) where you can be reached if we need to discuss your selection items. Carefully complete your list of database selections. Additional space has been provided to write any other considerations, which may be helpful to us in executing your selections. If a professional area is not listed on the printout, please indicate the type of information you are interested in, and we will do a personalized search to locate this information for you.

Calculating the cost
There is a $25.00 charge to perform the computer search for each area selected.   Example: If you requested information on National Hypnotherapy consultation firms, Child Development literature resource contacts and School Psychology Registry information, the cost would be 3 x 25.00 = $75.00, plus postage & handling.

What you receive
You will receive all matches that are consistent with the choices on your application form. The actual number of selections you will receive is contingent on the specific choices you make.
