How to use the Licensure & Certification Database

Filling out the order form
Fill out your complete name, address and telephone number(s) where you can be reached if we need to discuss your selection items. Complete your list of Licensure & Certification  selections. If you are requesting State-specific information, specify the State(s) next to each selection  Do not specify a State-specific search for a National program. Additional space has been provided for you to write any other considerations that may be helpful to us in executing your selections.

If a credentialling program is not listed on the database, please indicate the type of program you are interested in, and we will do a personalized search to locate this information for you.

Calculating the cost
There is a $10.00 charge to perform the computer search for each credentialling area selected, plus $3.00 for each geographical selection. Each national selection is considered as one geographical selection.

Example: #1: If you requested information on Employee Assistance Professional Certification, which is a National credentialling program, the cost would be calculated as follows: $10.00 for the computer search, plus $3.00 for the geographical selector (ie: national). Therefore, any requests for National credentialling programs would cost $13.00 each.

Example #2: If you requested information on Chemical Abuse credentialling, and you wanted this information specific to Utah, New Mexico, New York and New Hampshire, the cost would be $10.00 for the computer search, and $3.00 for each state group. Therefore, the cost would be $10.00 for the computer search, plus $3.00 X 4 state group. The total cost for this request would be $22.00.

What you receive
For each search requested, you will be sent a listing of all the appropriate credentialling programs. The actual number of listings will vary depending on the credentialling area selected.You will receive information listing the contact person, address, telephone number, the cost to become credentialled and a short description of the program.

If you would like the actual laws and application forms sent, please add $25.00 for each request.
